The purpose of this educational program is to educate regional providers on when to refer patients with heart failure for consideration for heart transplant and mechanical left ventricular support, to review other treatment options for complex heart conditions like aortic valve disease and infiltrative cardiomyopathies and ultimately lead to earlier referrals which will improve outcomes.
